I hereby affirm that I have no conditions that would make it unsafe for me or others to participate in the conference event.
I understand that the Grace Full Gospel Church staff may exclude me in the event that I fail to abide by the Grace Full Gospel Church health and safety protocols, disrupt, impede or interfere with the operations of the event, or threaten the health, safety or welfare of other participants or Grace Full Gospel Church staff, volunteers, ministers, etc. and that no refund will be made in such circumstances.
By signing this Waiver and Release of Liability, with full appreciation of the risk involved, on my own behalf, I hereby voluntarily release and forever discharge Grace Full Gospel Church, its trustees, officers, employees, volunteers, ministers, agents, insurers, guests, and contractors from any and all legal or financial responsibility for any personal injury, disability, illness, damage, medical expense or death, arising from or related to my participation in event. I agree, for myself to not to make any type of legal or equitable claim on Grace Full Gospel Church, or any the parties stated above with respect to any injury I may suffer, whether or not it arises through the negligence, omission, default or other action of anyone affiliated with Grace Full Gospel Church, including all parties mentioned above. I further agree that if any such claim is made, I will indemnify and defend Grace Full Gospel Church with respect to any such claim, injury or damage.
Medical Consent
I understand that Grace Full Gospel Church will make every effort to contact me in the case of an emergency. I give my permission for Grace Full Gospel Church to administer any medications needed and to provide and arrange for and consent to any necessary medical treatment for myself or any dependent in my care while at the event, including onsite and offsite emergency care. I accept responsibility for the costs of all such medical treatment.
Photograph Release
In consideration of my participation at the event, and without any further consideration from Grace Full Gospel Church, I hereby grant permission to Grace Full Gospel Church, staff and affiliates to utilize my appearance, performance or
voice in any and all manner and media throughout the world for the purpose of promotion, reporting or publication. Grace Full Gospel Church may use my likeness, voice and biographical material in connection with publication, promotion,
exhibition and distribution of such material. I understand that no royalty, fee or any other compensation of any kind shall become payable to me by reason of such release and use of any photograph.
